If any parent’s license is from out of state, please furnish an additional form of I.D., including but not limited to school I.D., state-issued I.D., work I.D., or expired driver’s license. We are able to accept cash, check, Visa, MasterCard, Discover, and American Express cards for payment. The Execution and Photo Fee(s) are payable to the Town of Henrietta. Do not include Execution or Photo Fees with the Application Fee! Please bring at least one (1) check or money order to be mailed along with your application for the Execution Fee and Optional Expedite Fee, made payable to the U.S. Department of State’s photo requirements.
